opens a slot the app first sends a GET, then it POSTs the file; both carry
the upload host's header set as the platform header model renders it (the
Android media-upload kind, the iOS media-upload family). The owner adds the
entity headers, and neither a caller's params nor the entity headers can
override the session headers.
The host is not Instagram's API: the HTTP layer keeps its answers away from
the session and from the Instagram error mapping (decided by the host), and a
non-2xx or an answer without the id is MessengerMediaUploadError.
