Routing-map (RMD) state reported on every request once the app has started evaluating its routing map:

• no-valid-map — evaluation started but neither the server map nor the default map is valid (rendered as the fail=…INVALID_MAP… header);
• url-eligible — the server map was accepted, so URL rewriting is eligible (rendered as the state=URL_ELIGIBLE header).
